You have it hosted somewhere (say, imgur, or tinypic).  Then, you take the link to the picture (should end with .jpg, .png, etc), and do this (using one random image from reddit as an example):

http://i.imgur.com/0OABovz.jpg

So, to embed the image, you put this at the beginning:

"[I MG ]"

Then, at the end of the link, you do "[ /img ]"

So like this:

[ IMG ] http://linkgoeshere.link [ /img ] (without spaces in brackets)
